Please include
- Facility name
- URL of the page (if you know it)
- What to correct or remove (the correct information is helpful too)
- Your role (optional: operator/staff of the facility, etc.)

To make confirmation smooth, contact from the facility’s official email, official site, or official phone is appreciated (to prevent impersonation, we may follow up with an additional check).
